TERMINATION NOTICE/DISTRIBUTION REQUEST FORM

INSTRUCTIONS: Your plan document details the timing of distributions for any terminating participant. When a terminated participant requests a distribution, please check your plan document before committing to a time of payment.
 
When a participant is eligible for payment, please complete and send a copy of the “Termination Notice/Distribution Request Form” to our office. Upon receipt of the completed form, we will calculate the vested benefit due and send you a distribution package. This package should be reviewed and signed by the trustee then passed on to the terminated participant for distribution election. Once the participant has made their election, please forward the package to our office. We will review the package to make sure that everything has been completed and send you a letter detailing the amount to be distributed and/or withheld.

PARTICIPANT LOAN APPLICATION

INSTRUCTIONS: Please read the loan policy and loan application forms carefully. If a participant requests a loan from the plan, please complete the “Participant Loan Application” and send it to our office for processing.
 
Our office will prepare the amortization schedule and supporting documents. At the same time, you must request the funds from your investment source for the amount of the loan.
